Sire or Dam's Registration number:  Exactly as it appears on registration certificate.
Organization:  If registration of Sire or Dam is by any organization other than American Purebred Registry please provide the name of the organization, and a photocopy of the registration certificate.  An individual registration form for each male and female in this letter will be issued to you upon acceptance of this application.  Liability limited to refund of registration fees.  APR has no affiliation with AKC, UKC, or CFA.
